New Patients
For your first appointment, please remember to bring a photo I.D., insurance cards, and a list of your current medications.
Radiological Studies ‐ To better serve you, please bring any recent (within one year) radiological studies including chest x‐rays or CT scans of the chest to your appointment. To obtain these studies, please contact the facility where it was done and request a copy of the films and the report associated with it.
Other Studies - If you have had a recent pulmonary function test and/or overnight sleep study, please obtain the results from either the physician who ordered it or the facility where it was done and bring it to your appointment.
For your convenience, our New Patient Packet is available for download below. You may choose to complete the forms prior to your appointment to save on time filling out paperwork once you have arrived.
Established Patients ‐ If you have information that needs to be updated (change in address, insurance, employment, etc), you may download and complete our Update Form and bring it to your next appointment.

Cancellation Policy ‐ If you are unable to make your appointment, please call us at 512-986-7765 to reschedule or cancel as soon as possible.
